Institutions make decisions with consequence.
They approve, refuse, sanction, appoint, remove, record, classify and certify.
Even a careful process can sometimes produce a wrong result.
The test of institutional integrity therefore does not end with the quality of the original decision.
An institution proves its integrity not only by making defensible decisions, but by possessing a credible mechanism for correcting decisions that later prove wrong.

If there is no credible path for review, a mistake can become permanent simply because the institution has already spoken.
Appeal, reconsideration, independent review and error-correction mechanisms exist because institutions are fallible.

Transparency does not always mean public disclosure, but it does require internal traceability.

Reversing a decision stops the wrong conclusion from continuing.
Remedy asks what damage the decision already caused.
Lost opportunity, financial effect, reputational harm or procedural disadvantage may require separate consideration.

When an institution gets it wrong
Institutional integrity is not the claim that every decision will be right. It is the capacity to detect, correct, record and repair when a decision proves wrong.
